2010 Childcare Fellowship Update
Applications:
- The deadline for the January 1 - December 31, 2010 award cycle was Friday, September 25, 2009.
- Applications for 2010 childcare expenses are no longer being accepted.
- The application period for 2011 will be announced in late summer 2010.
2009 Award recipients:
- The deadline to submit an FSA reimbursement form for 2009 expenses is March 31, 2010.
- If you chose an FSA for your Childcare Fellowship award:
Please fill out the 2009 FSA Reimbursement Form and fax it directly to Crosby Benefits with your receipt(s) as you
accrue your costs (i.e., on any date in 2009). Crosby will send a reimbursement check directly to you.

2010 Award recipients:
- If you chose an FSA for your Childcare Fellowship award:
Please fill out the 2010 FSA Reimbursement Form and fax it directly to Crosby Benefits with your receipt(s) as you
accrue your costs (i.e., on any date in 2010). Crosby will send a reimbursement check directly to you.
(The final deadline to submit an FSA reimbursement form for 2010 expenses is March 31, 2011.)
- If you did not choose an FSA for your Childcare Fellowship award:
Please fill out the 2010 Childcare Fellowship Verification Form and submit it to the HUCTW Office..
