2009 Childcare Fellowship Update

2008 Award recipients:

  • The 2008 information sheet is available at:
    2008 Childcare Fellowship Information.
  • If you chose an FSA for your Childcare Fellowship award:
    Please fill out the 2008 FSA Reimbursement Form and fax it directly to Crosby Benefits with your receipt(s) as you accrue your costs (i.e., on any date in 2008). Crosby will send a reimbursement check directly to you.
    If you prefer to receive the reimbursement via direct deposit, you can do so by submitting a direct deposit authorization to Crosby.
    Each pay period, 1/26 of your total award amount will be disbursed from the Childcare Fellowship fund into your FSA account. If you have also elected to add your own funds to a Dependent Care FSA, these will appear in the same FSA account. Whether the funds in your FSA account are from the Childcare Fellowship or from your additional payroll contribution, the same form is used to submit receipts to Crosby Benefits for reimbursement from this FSA account.

  • If you did not choose an FSA for your Childcare Fellowship award:
    Please fill out the 2008 Childcare Fellowship Verification Form and submit it to the HUCTW Office. This is the same process as in past years. You must fill out a quarterly verification form to receive your award. Quarterly deadlines are listed below.
