2012 Childcare Fellowship Application

The official deadline to apply for the 2012 Fellowship was September 30, 2011, but we will continue to accept late paper applications until Thursday, March 1, 2012. Please note that these late applications will receive smaller awards than those who got their applications in on time.

2012 Childcare Fellowship Verification Form

Please note that the reimbursement deadlines below are only for members who have already received 2012 Childcare Fellowship awards from us.

Verification forms are due to the HUCTW office according to the following schedule. Please submit your forms on time. Submit one verification form per provider for a three-month period (three months equals one quarter). Fellowship awards are delivered quarterly to your paycheck.
